Hydroclimatic changes in south-central China during the 4.2 ka event and their potential impacts on the development of Neolithic culture

The 4.2 ka event is widely presumed to be a globally widespread aridity event and has been linked to several episodes of societal changes across the globe. Whether this climate event impacted the cultural development in south-central China remains uncertain due to a lack of regional paleorainfall records. We present here stalagmite stable carbon isotope and trace element–based reconstruction of hydroclimatic conditions from south-central China. Our data reveal a sub–millennial scale (~5.6 to 4.3 ka) drying trend in the region followed by a gradual transition to wetter conditions during the 4.2 ka event (4.3–3.9 ka). Together with the existing archaeological evidence, our data suggest that the drier climate before 4.3 ka may have promoted the Shijiahe culture, while the pluvial conditions during the 4.2 ka event may have adversely affected its settlements in low-lying areas. While military conflicts with the Wangwan III culture may have accelerated the collapse of Shijiahe culture, we suggest that the joint effects of climate and the region's topography also played important causal roles in its demise.
Terrorist Attacks Against Emergency Medical Services: Secondary Attacks are an Emerging Risk

Introduction:
Terrorists increasingly aim at so-called soft targets, such as hospitals. However, little is known about terrorist attacks against Emergency Medical Services (EMS).
Objective:This study aims to review all documented terrorist attacks against EMS that occurred world-wide from 1970-2019 using the Global Terrorism Database (GTD).
Methods:Reports of terrorist attacks against EMS were extracted from the GTD from 1970-2019. Data collection included temporal factors, attack and weapon type, number of casualties, and if it was a primary or secondary attack (secondary attack: deliberate attack against the first responders of an initial terrorist attack). Reports were excluded if EMS were not a target or if it was unclear whether they were a target. Chi-square tests were performed to evaluate trends over time.
Results:There were 184 terrorist attacks against EMS, resulting in 748 deaths and 1,239 people injured. Terrorist attacks against EMS significantly increased over the past two decades. The “Middle East & North Africa” was the most frequently affected region with 81 attacks (44.0%) followed by “South Asia” with 41 attacks (22.3%). Bombings and explosions were the most common attack type (85 incidents; 46.2%) followed by armed assaults (68 incidents; 35.3%). Combined prehospital and hospital attacks were first reported in 2005 and occurred seven times. The first secondary attack against EMS dates from 1997, after which an increase was observed from 10 to 39 incidents in the periods 2000-2009 and 2010-2019, respectively.
Conclusions:This analysis of the GTD, which identified 184 terrorist attacks against EMS over a 50-year period, demonstrates that terrorist attacks against EMS have significantly increased during the years and that secondary attacks are an emerging risk. Bombings and explosions are the most common attack type. Terrorist attacks against EMS are most prevalent in countries with high level of internal conflicts, however, they have also occurred in western countries. These incidents may hold valuable information to prevent future attacks.

Background:
Analysts have warned on multiple occasions that hospitals are potential soft targets for terrorist attacks. Such attacks will have far-reaching consequences, including decreased accessibility, possible casualties, and fear among people. The extent, incidence, and characteristics of terrorist attacks against hospitals are unknown. Therefore, the objective of this study was to identify and to characterize terrorist attacks against hospitals reported to the Global Terrorism Database (GTD) over a 50-year period.
Methods:The GTD was used to search for all terrorist attacks against hospitals from 1970-2019. Analyses were performed on temporal factors, location, attack and weapon type, and number of casualties or hostages. Chi-square tests were performed to evaluate trends over time and differences in attack types per world region.
Results:In total, 454 terrorist attacks against hospitals were identified in 61 different countries. Of these, 78 attacks targeted a specific person within the hospital, about one-half (52.6%) involved medical personnel. There was an increasing trend in yearly number of attacks from 2008 onwards, with a peak in 2014 (n = 41) and 2015 (n = 41). With 179 incidents, the “Middle East & North Africa” was the most heavily hit region of the world, followed by “South Asia” with 125 attacks. Bombings and explosions were the most common attack type (n = 270), followed by 77 armed assaults. Overall, there were 2,746 people injured and 1,631 fatalities. In three incidents, hospitals were identified as secondary targets (deliberate follow-up attack on a hospital after a primary incident elsewhere).
Conclusion:This analysis of the GTD identified 454 terrorist attacks against hospitals over a 50-year period. It demonstrates that the threat is real, especially in recent years and in world regions where terrorism is prevalent. The findings of this study may help to create or further improve contingency plans for a scenario wherein the hospital becomes a target of terrorism.

Bacterial septic arthritis infections associated with intra-articular injection practices for osteoarthritis knee pain—New Jersey, 2017

Background:
In March 2017, the New Jersey Department of Health received reports of 3 patients who developed septic arthritis after receiving intra-articular injections for osteoarthritis knee pain at the same private outpatient facility in New Jersey. The risk of septic arthritis resulting from intra-articular injection is low. However, outbreaks of septic arthritis associated with unsafe injection practices in outpatient settings have been reported.
Methods:An infection prevention assessment of the implicated facility’s practices was conducted because of the ongoing risk to public health. The assessment included an environmental inspection of the facility, staff interviews, infection prevention practice observations, and a medical record and office document review. A call for cases was disseminated to healthcare providers in New Jersey to identify patients treated at the facility who developed septic arthritis after receiving intra-articular injections.
Results:We identified 41 patients with septic arthritis associated with intra-articular injections. Cultures of synovial fluid or tissue from 15 of these 41 case patients (37%) recovered bacteria consistent with oral flora. The infection prevention assessment of facility practices identified multiple breaches of recommended infection prevention practices, including inadequate hand hygiene, unsafe injection practices, and poor cleaning and disinfection practices. No additional cases were identified after infection prevention recommendations were implemented by the facility.
Discussion:Aseptic technique is imperative when handling, preparing, and administering injectable medications to prevent microbial contamination.
Conclusions:This investigation highlights the importance of adhering to infection prevention recommendations. All healthcare personnel who prepare, handle, and administer injectable medications should be trained in infection prevention and safe injection practices.

Introduction
Early identification of traumatic brain injury (TBI) is essential. Near-infrared spectroscopy (NIRS) can be used in prehospital settings for non-invasive monitoring and the diagnosis of patients who may require surgical intervention.
MethodsThe handheld NIRS Infrascanner (InfraScan Inc.; Philadelphia, Pennsylvania USA) uses eight symmetrical scan points to detect intracranial bleeding. A scanner was tested in a physician-staffed helicopter Emergency Medical Service (HEMS). The results were compared with those obtained using in-hospital computed tomography (CT) scans. Scan time, ease-of-use, and change in treatment were scored.
ResultsA total of 25 patients were included. Complete scans were performed in 60% of patients. In 15 patients, the scan was abnormal, and in one patient, the scan resulted in a treatment change. Compared with the results of CT scanning, the Infrascanner obtained a sensitivity of 93.3% and a specificity of 78.6%. Most patients had severe TBI with indication for transport to a trauma center prior to scanning. In one patient, the scan resulted in a treatment change. Evaluation of patients with less severe TBI is needed to support the usefulness of the Infrascanner as a prehospital triage tool.
ConclusionPromising results were obtained using the InfraScan NIRS device in prehospital screening for intracranial hematomas in TBI patients. High sensitivity and good specificity were found. Further research is necessary to determine the beneficial effects of enhanced prehospital screening on triage, survival, and quality of life in TBI patients.

Stretchable electronics represent an emerging class of devices that can be compressed, twisted and conform to very complicated shapes. The mechanical and electrical compliances of the technology promise to open up applications for healthcare, energy and entertainment purposes. However, advancement in the field has been hindered by material related constraints. Moreover, the current microfabrication facilities are optimized for rigid substrates such as silicon, which have significant different properties compared to elastomers. In this paper, four categories of enabling technologies for stretchable electronics commercialization are critically reviewed, namely: the novel design of stretchable structures, use of non-conventional materials, state-of-art printing techniques and also the patterning of electrodes or metal interconnects via conventional manufacturing techniques.

Are mangroves worth replanting? The direct economic benefits of a community-based reforestation project

Competition for coastal land use and overexploitation have reduced or degraded mangrove coverage throughout much of their distribution, especially in South-east Asia. Timber production was the initial motivation for early mangrove reforestation projects. More recently, benefits from protection against erosion and extreme weather events and direct improvements in livelihoods and food security are perceived as justifications for such restoration efforts. This study examines the socioeconomic impacts of a community-led reforestation project in the Philippines through a survey of the local fishers. Revenues from mangrove fisheries, tourism and timber result in an annual benefit to the community of US$ 315 ha−1 yr−1. This figure is likely to be considerably more if the contribution of the mangrove to the coastal catch of mangrove-associated species is included. This estimate only includes direct benefits to the community from mangroves, and not intangible benefits such as coastal protection, which paradoxically is perceived by the community as one of the most important functions. More than 90% of all fishers, regardless of where they fished, thought the mangrove provided protection from storms and typhoons and acted as a nursery site and should be protected. Those fishing only in the mangrove perceived more benefits from the mangrove and were prepared to pay more to protect it than those fishing outside. This study concludes that replanting mangroves can have a significant economic impact on the lives of coastal communities. Acknowledgement of the value of replanted mangroves compared with other coastal activities and the benefits they bring to the more economically-vulnerable coastal dwellers should support better informed policy and decision-making with regard to coastal habitat restoration.

There is considerable dispute over the value of frontal leucotomy in the treatment of mental illness. Uncontrolled studies have been numerous. Many report good results (Pippard, 1955; Scoville, 1960; Sargant and Slater, 1963; Birley, 1964; Sykes and Tredgold, 1964; Post, Rees and Schurr, 1968). Some are more sceptical (Barahal, 1958). Controlled investigations have been few, and their findings unfavourable. Robin (1958) observed no difference between the outcome of 198 patients who had a standard leucotomy for functional psychosis and an equal number of carefully, but retrospectively, matched controls.
